1、領洗申請表格

2、領洗者居民身份證（副本）

3、領洗者出生證明書，民事登記局發出：

• A) 澳門出生申請（出生登記之敘述證明）
• B) 外地出生申請（身份證明局之證明書Certidão ）

4、領洗者地址證明

5、代父/母 居民身份證（副本）

6、代父/母 領洗及堅振證明（要3個月有效期）

7、結婚證明書（註：如領洗者已結婚要效這證明書）

8、父母 領洗紙及結婚證明（註：如領洗者是3歲以下要交這證明書）

1. Baptism form

2. Baptize Macau ID Card (copy)

3. Baptize birth certificate, issued by the civil Registration office

• A) Born in Macau (Descriptive proof of birth registration )
• B) Born out of Macau (Certificate from the Identification Bureau ) (Certidão )

4. Baptize address proof

5. God father/mother’s ID Card (copy)

6. God father/mother’s Baptist and comfirmation certificate. (Valid within 3 month )

7. Marriage Certificat ( Only if Baptize is Married )

8. Parents Baptist and Marriage Certificate. ( Only if Baptize is under 3 years old )
